local nursing homes. Whether patients become part of
Hospice before or after entering a nursing home,
they receive the same excellent care they would in
their own homes.

at the end of life. These combined strengths of
nursing-home staff and of Hospice staff create
quality, compassionate care for nursing-home
patients and their families.
Hospice supplements nursing-home care by offering these

- A registered nurse serving as Hospice’s
nursing-home coordinator
- Specially-trained nurses implementing home care, pain management,
and symptom control
- Personal-care specialists assisting patients with
basic hygiene
- Social workers providing psychological and
emotional support to patient & family
- Pastoral-care specialists consulting with patient
and family members about spiritual concerns in
conjunction with the patient’s own minister or
priest
- Trained volunteers offering companionship and
comfort
- Bereavement specialists providing grief
counseling and support during and after the
